Information About Your Intake Appointment

Prior to your intake appointment, please print out and complete the forms at the bottom of this page. Bring the completed forms with you to the intake. If you cannot print the forms, let us know ahead of time and they will be provided for you during the appointment. The Policy and Procedures form describes our treatment policies. Please read this form thoroughly and ask any questions at the time of your appointment. Bring the signed last page with you to the appointment indicating that you consent to treatment. Follow the directions on the remaining forms and fill them out as completely as possible.

The intake will take approximately 45 minutes to one hour. Most of the time is spent interacting with your child and the rest of the time is spent discussing whether your child is a good fit for our services. Your child may be in the room during our discussion so if there is anything of a private nature that you would like us to know, please include it on these forms or e-mail the information ahead of time.

Payment is due at the time of the intake. Cash or check only. An additional deposit may be necessary to hold your child's spot in an upcoming group, if applicable. Please provide as much advance notice as possible if you must cancel or reschedule your appointment.
